Crossed Cerebellar and Contralateral Thalamic Hyperperfusion in Epilepsy

چکیده
A 59-year-old man with no history of seizure, who was undergoing treatment for chronic alcoholic cirrhosis, was admitted for a fever due to cellulitis and alcohol withdrawal. A generalized seizure with left facial twitching developed after the patient lost consciousness during status epilepticus (SE). Electroencephalogram showed periodic lateralized epileptiform discharges (PLEDs) over the right hemisphere (Picture A). The seizure stopped after intravenous administration of midazolam. Three days after SE onset, arterial spinlabeling perfusion magnetic resonance imaging, a noninva-

Interictal crossed cerebellar hyperperfusion on Tc-99m ECD SPECT.
Crossed cerebellar hyperperfusion (CCH) in epilepsy is a rare condition that is observed on ictal cerebral perfusion SPECT. The mechanism of CCH assumes that hyperperfusion in the epileptic foci of the unilateral supratentorium causes hyperperfusion secondary to the corticopontocerebellar pathway (CPCP)-mediated remote effect in the contralateral cerebellar hemisphere. This phenomenon is simila...
متن کاملCrossed cerebellar glucose hypermetabolism demonstrated using PET in symptomatic epilepsy--case report.
A 65-year-old male with an old cerebral contusion in the frontal lobes had suffered from status complex partial seizures. Ictal positron emission tomography with an F-18 fluorodeoxyglucose ([18F]FDG-PET) scan revealed hypermetabolism in the frontal, temporal and parietal lobes and the contralateral cerebellar hemisphere. The patient underwent a next-day PET scan with the 15O-labeled gas inhalat...
متن کاملTransient crossed cerebellar diaschisis secondary to cerebral hyperperfusion following carotid endarterectomy.
We report a case of transient crossed cerebellar diaschisis secondary to cerebral hyperperfusion following carotid endarterectomy. The appearance of crossed cerebellar diaschisis under the presence of cerebral hyperperfusion may suggest the development of hyperperfusion syndrome.
متن کاملCrossed cerebellar diaschisis secondary to refractory frontal seizures in childhood
We report a girl with refractory partial seizures since 7 years of age, secondary to right frontal cortical dysplasia, who developed MRI and SPECT abnormalities in the contralateral hemicerebellar cortex. These became more marked, leading to left hemicerebellar atrophy. Crossed cerebellar diaschisis has been described mostly in hemispheric stroke and supratentorial tumours, but less often in ep...
متن کاملMR imaging volumetry of subcortical structures and cerebellar hemispheres in temporal lobe epilepsy.
BACKGROUND AND PURPOSE There is mounting evidence of extratemporal volume changes associated with medically refractory temporal lobe epilepsy (TLE). This MR imaging study aimed to characterize volume changes in subcortical structures and cerebellar hemispheres with respect to lateralization of the seizure focus, onset and duration of epilepsy, and frequency of generalized tonic-clonic seizures ...
